In a post from the Jefferson County WI Government, they have provided a survey for those who fall into phase 1B of the COVID-19 vaccine so they can indicate their interest in the vaccine. If you’re interested in receiving the vaccine but do not have access to the survey, you can call the Health Department at 920-674-7275. More information, including who falls into phase 1B and who is prioritized within the phase can be found on the Jefferson County WI Government Facebook page.
