According to a press release from the Beloit Area Community Health Center, they have received Patient-Centered Medical Home Recognition through the National Committee for Quality Assurance. The Patient-Centered Medical Home program recognizes clinics that deliver high quality, patient-centered care that focuses on sharing information with the primary care clinician and other providers who work as a team to coordinate care by referring other services within the Health Center, community Resources, and secondary services. With this method of care, providers can focus on the care of their patient and can also assist in troubleshooting issues patients have with getting prescriptions filled, obtaining transportation, and navigating other healthcare barriers.
