Jefferson County Shares New COVID-19 Vaccine Survey

According to a post from the Jefferson County Wisconsin Health Department, they have created a new COVID-19 related vaccine survey. For many different reasons, sometimes the health department has leftover vaccines at the end of the day that would otherwise go to waste if not used. This survey is for residents to fill out if they can come on short notice to receive a COVID-19 vaccine that otherwise wouldn’t be used. This is not the same survey that is used for regularly scheduled vaccine appointments. For more information, including the survey, see the Jefferson County Wisconsin Health Department Facebook page.
