COVID-19 Vaccine Requires Attestation Form in Watertown

In this week's Weekly Roundup on the City of Watertown WI Government Facebook page, Mayor Emily McFarland discussed that individuals who are eligible to receive their COVID-19 vaccine must sign an attestation form before getting the shot. Signing the form confirms that you are eligible and wish to receive the vaccine. The form can be found on the City of Watertown WI Government website, and must be brought with you to your appointment.
